Medical Records Request

NEVHC values your health information and privacy. Find out how to access, request, and securely share your NEVHC medical information.

Requesting your NEVHC medical records:

NEVHC’s patient portal:

This is the best way to access your NEVHC medical or dental information, as well as to request your NEVHC medical records. It’s convenient, easy, and free!

This portal is a secure HIPAA-compliant website for you to view your medical information, communicate with your care team, review diagnostic test results, and more.

On the patient portal, you can access most of your health information for free, and you can view or download those parts of your record. For information that’s not available, you can request it via the portal. Clerks will upload the requested records to your portal for free.

If you don’t have a patient portal account, find out more about the benefits of the patient portal and how to sign up below.

Other ways you can request your medical records:

Mail or submit your request to the Medical Records department of your NEVHC health center or fax your request to our Medical Record Coordinator’s office at (818) 270-9271.

For paper records, there are costs for obtaining a copy of your information:
  • NEVHC charges a $15 administrative fee plus a copying fee of 25 cents per page up to 200 pages, after which the fee is 12 cents per page. The maximum fee is $400.
  • We provide patients up to five pages of copies at no charge.
  • If you need your medical or dental information released to support a denial of a claim for public benefits, NEVHC will release the relevant portion of your record for your appeals case at no charge. Fees may apply if your request is being handled by an attorney’s office who is requesting records on your behalf.

Requesting medical records be sent to another provider:

If you are being referred by NEVHC to a specialist:

NEVHC will automatically send relevant information about your care or treatment to that provider so long as you have verbally consented to the referral.

If you are requesting that your health information be released to other providers or third parties:

  • You must authorize the release of this information in writing by completing an Authorization to Release Information form.
  • You can print the Authorization to Release Information form or fill it out electronically. If you fill out the form electronically and use an electronic signature, please provide a copy of your ID.
  • Return the form to the Medical Records department of your NEVHC health center or fax the form to our Medical Record Coordinator’s office at (818) 270-9271.
