How can the Patient Portal help me?

The patient portal is a secure online website and mobile app that gives you convenient, 24-hour access to your personal health information. The patient portal can help you do the following: 

Request appointments

Ask for medication refills

Check lab results

Send messages to your
Provider or Care Team

Get appointment reminders

How do I sign-up
for the patient portal?

Step 1:
Go to: login.intelichart.com/nevhc

 

 

Step 2:
Register your account by using an existing email account. You’ll need to have your PIN or verification question on-hand to complete this step.

 

 

Step 3:

Complete the process by confirming your email address for future logins. Please store your login information.

Frequently asked questions

Within 24 hours you will receive an enrollment link to the email you provide on the day of your visit. If you don’t receive an enrollment link, please call the Patient Portal Hotline at (818) 979–7444 to request a PIN.

Registration Guide

Yes, it is now easy to add a patient to your account by requesting a PIN. There are no limits of the number of patients you can add to your account.

How to Add Child Guide

Yes, you can grant access to those who are caring for you and overseeing your health care by providing us their information in the patient form packet. With your permission, they will be able to request non-urgent future appointments, request prescription refills, have access to your medical information and handle other healthcare needs.

You will be able to ask your Provider and/or Care Team any non-urgent medical questions about your medication, referral status or healthcare issues.

NEVHC follows strict HIPPA practices to pretect your information. The data on your Patient Portal is safe as long as you do not share your log-in information with others.

Due to HIPPA regulations, we are unable to provide sensitive results such as STD, HIV, HEP C, Gonorrhea, Pap Smear and Biopsy results. The laboratory results mentioned must be requested in person or request to schedule an appointment.

Please send us a message or call your home clinic to update your patient information.

Please call the Patient Portal Hotline at (818) 979–7444 for any technical issues or to have your account reset.

Due to Patient Privacy laws a parent or legal guardian are prohibited from accessing certain types of medical information about a minor age 12 or older without specific consent of the minor. Due to these legal requirement, full access to the patient portal will be given to the patient and only if the patient consents, will full patient portal access be given to the parent/guardian.

NEVHC is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n) with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
