During the month of April, we celebrate National Public Health Week (April 4 – 10). During this week, we recognize the impact of public health on our community, and highlight the issues that need to be addressed to continue improving our community’s health. We have wonderful public health professionals at NEVHC who ensure you have the necessary tools and resources to stay healthy, and can help you make healthy lifestyle changes to improve your family’s health.

 

Staff Spotlight: Carolina Aguilar, Program Manager of Health Education

As part of National Public Health Week, we are pleased to spotlight Carolina Aguilar, a member of our Health Education department. Health educators are an essential part of the public health team because they help empower individuals and communities to live healthier lives. In 2015, Carolina Aguilar joined NEVHC as a Health Educator facilitating health education classes. Now the Program Manager of Health Education, she oversees and promotes health education programs within our health centers. Carolina grew up in the same community she now works with, and was an NEVHC patient as a child. She states, “It is very rewarding to have the opportunity to be able to work with our community and provide these health education classes/programs that help them make lifestyle changes to better their health.”

 

NEVHC Wins P4 Challenge

The U.S. Department of Health & Human Services (HHS), through the Health Resources and Services Administration (HRSA) announced NEVHC as a winner of the Promoting Pediatric Primary Prevention (P4) Challenge! Because of the ongoing pandemic, there was a decrease in well-child appointments and vaccinations. To keep our pediatric patients safe and up to date on their vaccines, our providers made personalized phone calls and video messages to explain the importance of vaccines, and what parents can expect at their child’s upcoming appointment. Make sure your children have a healthy start; ask their doctor if they are up to date on their vaccines and routine appointments. To view one of the videos we created click here.

 

NEVHC Receives Captains of Industry Award

NEVHC recently attended the Greater San Fernando Valley Chamber of Commerce’s 111th birthday, where we were honored with the Captains of Industry Award for the healthcare industry! This honor is awarded once every five years, and we are so thankful for this recognition. We are thrilled to have been a resource for our community since 1973, and we are excited about our growth and innovations.

 

How To: Request Your Medical Records

As a patient, you have the right to see and receive copies of your medical records maintained by NEVHC. NEVHC values your health information and privacy. We will never release or otherwise disclose information about you or your health without your permission unless we are permitted by law.

If you are being referred by NEVHC to a specialist, we will automatically send relevant information about your care or treatment to that provider, so long as you have verbally consented to the referral. If you are requesting your health information to be released to other providers or third parties, you must authorize the release of this information in writing by completing an Authorization to Release Information form. You may download the form here.

 

If you would like to request your medical records, please use one of the following methods:

 

· MAIL request to: Northeast Valley Health Corporation, 1600 San Fernando Rd., San Fernando, CA 91340

· FAX Requests to: (818) 270-9271

· PRESENT a subpoena or court order to: 1600 San Fernando Rd, Bldg A, San Fernando, CA 91340—ask for the Medical Records Program Coordinator.

· EMAIL questions: medicalrecords@nevhc.org

· If you have access to the NEVHC portal, you may access your record at no cost, and you may view or download those parts of your record. You can also request medical records to be sent to you via your patient portal.
