Cooperative Health Care for the Homeless Network

According to the 2020 Greater Los Angeles Homeless Count, there are approximately 66,436 individuals experiencing homelessness throughout Los Angeles County. For people who live on the streets, in cars, and emergency shelters, homelessness takes its toll—not only on the physical body but on the human spirit as well.
 
NEVHC believes that all residents, even those without a permanent address, have the right to quality healthcare. Therefore, as the lead agency of the Cooperative Health Care for the Homeless Network (CHCHN), we are proud to collaborate and supplement the funding of a group of 12 compassionate and dedicated service providers who serve homeless residents throughout Los Angeles County. The Network provides approximately 22,000 homeless men, women and children with primary health care each year. Services include preventive care, treatment of acute and chronic health conditions, dental care, case management, behavioral health care, alcohol/substance abuse treatment, including medication-assisted treatment (MAT), referrals for needed social services and specialty care, outreach, as well as health insurance enrollment assistance. No one is turned away due to their inability to pay.

In operation since 1987, the Network was originally funded as one of 19 demonstration projects privately funded by the Robert Wood Johnson Foundation and the Pew Charitable Trusts with a focus on primary medical care and case management. In 1988, Title VI of the Stewart B. McKinney Homeless Assistance Act created the Homeless Health Care (HCH) program under Section 340 of the Public Health Service Act. The McKinney Act also provided funding for programs such as emergency food/shelter, transitional/ long-term housing, mental health services, and education/job training. In 1993, supported by the Bureau of Primary Health Care, under Section 330(h) of the Public Health Service Act (with Community and Migrant Health Centers), the Bureau of Primary Health Care funded 10 Homeless Health Care (HCH) outreach projects for homeless children and their families. In 1996, the Health Center Consolidation Act put HCH programs under Section 330(h) of the PHS Act (with Community and Migrant Health Centers).

Los Angeles County is composed of eight (8) health service planning areas (SPAs) and NEVHC has CHCHN service providers serving homeless residents in all eight of these SPAs.
