What if I do not have health insurance or cannot afford my deductible?
Northeast Valley Health Corporation offers services on a sliding fee scale for individuals and families with limited income. Eligibility is based on family income and family size. Certified Enrollment Counselors are available at our health centers to help you determine if you are eligible to pay for services based on the sliding fee discount program .

To apply to the Sliding Fee Discount Program your Certified Enrollment Counselor will ask you to download and sign the Sliding Fee Discount Program application linked below and do one of the following:
- Email forms, copy of income, ID to insurancehelp@nevhc.org with the subject line: “Staff Name and Location”. Please note: No method of transmission over the Internet is 100% secure and we cannot guarantee absolute security
- Fax forms, copy of income and ID
- Bring in completed forms to your scheduled in-person visit
