Distributed over 1,000 hygiene and essential clothing items to patients experiencing homelessness

WIC Breastfeeding Program

Improved exclusive breastfeeding rates by distributing 379 nursing bras and 153 nursing aprons to 532 new mothers

Breathe Easy Asthma Program

Distributed 300 asthma-safe cleaning kits to 300 patients attending health education asthma classes

Breast Cancer Awareness

Purchased promotional materials to promote Breast Cancer Awareness across our 10 Women's Health locations

Home BP Monitor Program

Increased blood pressure control rates by purchasing 140 unconnected blood pressure monitors for patients with hypertension

Reach out and Read

Distributed 7,666 books to 4,501 pediatric patients during their well-child appointments

Cuddles of Love

Distributed 530 baby clothing sets and 21 strollers to new mothers

ECM: Supporting Healthy Habits

Purchased 60 organizational tools to support enrolled patients with appointment management and medication adherence

FIT KIT Mail Out Program

Increased colorectal cancer screening rates by mailing out 847 FIT Kits and screening 387 patients

Keeping Tiger Teens Safe

Purchased 427 fun incentives for San Fernando High School students who received a health screening

Harvesting Health

Distributed 1,400 health education materials, such as healthy portion plates, water bottles and produce tote bags

Healthier Living

Increased health education participation rates by providing 255 attendees with items such as cutting boards, portion bowls and grocery tote bags

Lactation Awareness

Assisted 100 mothers in breastfeeding with new lactation education materials

Little by Little

Assisted 1,100 WIC families in bringing books home to begin their personal libraries

Meal in a Card

Purchased 350 meal gift cards to distribute to patients living with HIV/AIDS and impacted by homelessness
