pathway to wellness

The expansion of our Pathway to Wellness engraving opportunities is underway! Leave a commemorative tribute or memorial gift in honor of your organization, a colleague, or loved one by personalizing a bench plaque or brick engraving. You can help us build a healthier and fairer community while leaving your permanent mark on the Pathway to Wellness at the San Fernando Health Center campus. All who use the pathway will learn the name of the businesses, organizations and individuals that contribute to the high-quality care that NEVHC provides.

SECURE YOUR COMMEMORATIVE TRIBUTE

To see your engraving on the Pathway before the end of the year, reserve your tribute by October 31, 2021. Contributions are tax deductible, with the exception of engraving fees.

 

NEVHC employees may also reserve their tribute by downloading and filling out the pledge form.

NEVHC is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n) with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
